For your first appointment with us, we ask that you please arrive 10 minutes prior to your appointment time to fill out some new patient paperwork and provide insurance information. However, if you would like to save some time in the waiting room, we have the same forms we would ask you to fill out available below.
Please bring the following to your first appointment:
- Insurance card
- Photo ID
- Accurate list of current medications (names and dosages)
Below are some forms that all first-time ActiJoint patients need to fill out prior to their initial evaluation. Please feel free to download, print, and fill out these forms before coming in if their requirement applies to you. Otherwise, we will have them ready for you to fill out at our office.
